How many rooms or televisions do you want to connect to your satellite system?

Choose the system that you think is right for you. Think about how many rooms will have DISH Network and if you want a DVR, an HD system, or both. When you speak with an installation professional or service representative they will confirm your equipment preferences. The receiver and programming choices you make on this order form do not need to be your final choices. Room selections needing multiple receivers can result in additional receiver and monthly fees.

3. AGREEMENT
With 24 Month agreement, $99 one time activation fee will be waived. If no agreement taken, $99 one time activation fee will apply.

Programming Package credits.

Programming Package credits: requires a 24-month Agreement and qualifying programming. Must maintain qualifying programming to receive all credits. Upfront Credit: receive a credit for each of the first 12 months after service activation. Online Bonus Credit: when you create a DISH Network online account, agree to AutoPay, and opt-in to the DISH Network E-Newsletter, receive an additional Online Bonus credit for each of the first 12 months after offer redemption. You must redeem no later than 45 days from DISH Network service activation. Must maintain continuous enrollment in AutoPay.

If qualifying service is terminated prior to end of 24 months, a cancellation fee equal to the lesser of $420 or $17.50 per month for each cancelled month of service will apply.
